Initial Visit Items

Prior to your initial visit we need for you to do a few things that will be helpful to you and us. We would like for you to:

Bring a list of any prescription or over-the-counter medications you are currently taking, along with the dosages for each.
If you have health insurance you would like us to file, please bring your insurance card.

On your initial visit to Watauga Surgical Group, you will be requested to fill out a variety of forms pertaining to your medical history and current medical condition. We will also provide you with some forms informing you of our policies and procedures.

To save you time, we have made our forms available for you to view and/or print. You may fill them out and bring them with you on the day of your appointment. Our office staff will be happy to answer any questions you may have about the forms.

Waiting Room Check-In

To print the forms, click on each link to the left, one at a time. Each click will open up a new browser window with that form in it (in PDF format). Just click the Print icon and it should send the form to your printer. You will need the free Adobe PDF Reader to view the form, and you can get it by clicking here.
